I just dropped and broke an entire vial of a controlled substance. What reporting is needed?
The first thing that you need to do is to clean up the spill. Liquids should be absorbed with a compatible material (paper towels are acceptable for most materials). The absorbent should then be placed in a plastic bag and sealed. Solids should be collected similarly, wiped up with a paper towel and bagged. (Be careful of broken glass. Wear proper gloves.) Once the spill has been cleaned up, the loss needs to be noted on the inventory log sheet for the agent involved. The entry should be initialed by the person handling the spill as well as by someone who witnessed the spill (if applicable). The DRC must handle disposal of the material.
