I’m in UK and split with my son’s father when he was 6 months old (we were not married). I wanted to change my son’s surname back to my maiden name and made an appointment to see a solicitor. I couldn’t change the birth certificate and had to do a Change of Name Deed. I didn’t pay anything at that time as I was receiving income support and it was quite a few years ago.I do know that if you are married or even divorced, you do have to have the father’s consent. Hope this helps.
