• When a parent requests a re-evaluation, use the Re-evaluation Worksheet instead of Record of Parental Request for Re-evaluation form. However, be sure to document the date of parental request. If the parent has made a request in writing, attach this to the Worksheet. • If a student needs a re-eval for continued eligibility for SI or LI and their primary disability is not LI or SI, what forms do the SLPs need to use? What would be the best way to tell the TOR that we need to conduct our re-eval before the next case conference? The TOR must complete the Re-evaluation Worksheet (checking Option 2) at the case conference and provide it to the SLP. The SLP will then provide the TOR with the appropriate paperwork to collect information from the TOR and general education teacher. This must be returned to the SLP within 3 school days.
