If a physician meets the CME waiver by either completion of fellowship within the past three years or certification by a relevant board, how is this documented in the online application?

To indicate in the online application system that a physician meets the CME waiver simply select edit next to the staff member’s name under the Manage Staff link, scroll to the bottom of the screen and enter the reason for the waiver in the “course name” field. In the completion date field, you enter the date exam or fellowship completed and then enter 15 for the number of hours into the “ICANL” field. For more information on CME/CE waivers see ICANL Standard A1.3.4 and A1.4.3.
