If a provider’s software is not updated by April 1, 2009, will claims be rejected if we continue to list UPN information in Remarks field (Box 19) of the CMS-1500 claim form?
For dates of service on or after April 1, 2009, the UPN qualifier and UPN must be entered, starting in the shaded area located under the Date(s) of Service field (Box 24A), left justified, on the CMS-1500 claim form. The Unit of Measurement Qualifier and the 10-digit quantity must be entered in the shaded area under the Procedures, Services, or Supplies field (Box 24D) on the claim form.
