State agencies may submit lump sum notifications in two ways. Visit the Office of the Attorney General’s Employer website to learn more about the options listed below. • Electronic Lump Sum Payment Notification Form – The online form allows employers to enter their company’s information and up to 250 employee records at a time. This form is available to all employers and does not require website registration. Registered users do not need to log in to the site to use the form. Click on “Intent to Pay Lump Sum” under Online Reporting and then select “Launch the Notice of Intent to Distribute Lump Sum Payments Form.” • Data Transfer System (DTS) – A spreadsheet is available in the “Employer Information Center” under “Forms” section of the Employer website.
