If you will continue as a dual Motorolan in 2010 and you will not be the primary health plan coverage member, you do not have to complete the Questionnaire by the deadline (refer to Question #2 for more details) to pay lower contributions. Remember, as a primary health plan coverage member, if you elect a Motorola Health Plan in 2010 and do not complete the Health Risk Questionnaire by the deadline, you will pay the regular contribution rates not the discounted contribution rates in 2010.
