Im setting up a database and I want a title index. Can I make one for myself or do I have to use dc.title?

It’s much preferred that if an index exists with the correct semantics, then servers should use it rather than inventing new ones. That said, there’s nothing to stop you from creating an index set with a different name and identifier but with the same semantics as others. Just realise that it’s not necessary and will make it harder to interoperate with your database as people will want to send dc.title for a title search.
