Is it true that the facility requesting the contact hours must furnish the certificates?
Yes. This office does not maintain certificates. The standard one used is the DA87 Certificate of Training which is available through your supply office. You can also develop/create one of your own but if you do a copy must accompany your application (see examples). The program administrator or her/his appointee will sign the certificates.
