Is the Catholic Church the true Church, as founded by Jesus Christ?

When you say Catholic Church you need to be more specific. There is an original Catholic Church and there are the Roman Catholic Church; the Orthodox Church, which also has branches; and there is the Anglican Church which is a Catholic Church and has branches in the UK and the British Commonwealth and Episcopalian elements in North America. Which is it do you mean? The Roman Catholic Church - The Council of Trent defined that the sacraments, of which they hold there are seven, of the "New Law" of the Church, were instituted by Christ (Sess.VII, can i). The Roman Catholic Church holds that God alone is the principal cause of the Sacraments. He alone authoritatively and by innate power can give to external material rites the power to confer grace on men (Cath. Encyc. Sacraments, Vol.XIII, p. 298).
