Is the foreign workers name required on the Prevailing Wage Request form?
The foreign worker’s name should be provided when the prevailing wage determination will be used to support an Application for Permanent Employment Certification. Although the foreign worker’s name is not required to obtain a prevailing wage for H-1B requests, a name or other identifier helps us in tracking the request in our database if the firm has multiple requests.
