Ans. Yes, there are prescribed forms i.e. Form No.1 for birth reporting, Form No. 2 for Death reporting, Form No. 3 for still Birth reporting, Form No. 4 for reporting medical certification for cause of death (institutional) and Form 4A for medical certificate for cause of death (non-institutional).
