T3. 2.7.5 Regarding adenocarcinoma of the lung with a predominantly bronchioloalveolar pattern of growth and a central area of scarring/invasion. Is the measurement of the tumour made on the overall size or is the T category based upon the greatest dimension of the invasive component only? Our pulmonary pathology consultant stated: Tumour size is based on total size – not just the invasive component. There are studies demonstrating prognostic importance of the size of the invasive component – but for staging purposes we use the total tumour size including the bronchioloalveolar component. For bronchioloalveolar carcinoma without invasion, tumour size is based on total size.
