Yes, this Declaration is required by CCP 587 when seeking entry of DEFAULT. Absent filing of this declaration, the clerk may not enter defendants default. The Declarant should remember to sign the declaration, enter the date, and check the appropriate box under paragraph 6. If applicable Declarant should provide the last known addresses for each of the defaulted defendants under paragraph 6 (b).
