If your child is on straight Medical Assistance (MA), call Public Health at 952-361-1329 and ask for the PCA coordinator. If your child is on a Prepaid Medical Assistance Program (PMAP), call your health plan using the number on the back of your card. If you need to apply for Medical Assistance, call Community Social Services 952-361-1600.
