My child was covered under UCs plan until May 2010 when he/she turned age 23. Do I need to fill out a form to add him/her back to my coverage or will he/she automatically be added?

My child was covered under UCs plan until May 2010 when he/she turned age 23. Do I need to fill out a form to add him/her back to my coverage or will he/she automatically be added?

You must complete a form and submit it to UC Benefits no later than November 30, 2010. Previously covered individuals will not be automatically “re-added” to UC’s health plans.
