If you want to do it at Fidelity start by having them switch your 401(k) there to an IRA account. Once that is an IRA, from the Customer Service tab, Overview, click the link for “Transferring Assets to Fidelity. You may need to mail or deliver the form Fidelity to transfer assets from your old IRA to the Fidelity IRA (they can handle the transfer from their end). You have a choice of leaving investments in the funds they are in (transfer in kind), or selling them for cash instead for the transfer. There is no tax or withholding for a direct trustee to trustee transfer.
