Should I send a Child Information Form (green sheet) to the WECA CACFP office?

No. The information you would ordinarily put on the green sheet must now be e-mailed to WECA CACFP. This includes: Updates to Child Information (to drop a child from care, to correct a date of birth, etc.), School-age Child in Care, Holiday Care, Change in Provider Information, Infant Food Provided by Parent, Email Address Change, and Withdrawal from the Food Program. Go to the WECA website at www.wecanaeyc.org. Click on Food Program, then On-Line Forms, and select the form you need. Carefully complete all required fields, scroll to the bottom of the form and Send. Your information will be automatically sent to WECA CACFP.
