Some slight changes were made to the dental benefit. Dental coverage will increase the covered cleanings from two to four per year. If the Health Assessment is completed the annual in-network maximum benefit under the dental plan will increase from $1500 to $2000 for each covered person enrolled in either the Self-Funded PPO plan or the HMO plan. No changes were made to the long term disability (LTD) benefit, life insurance benefit, or accidental death and dismemberment (AD&D) benefit.
