What are Appalachians immunization requirements?
In order to comply with North Carolina law (General Statute 130A 152-157), the University requires that all entering students submit a complete record of immunizations. The statute requires the University to withdraw administratively students with incomplete or missing records. Please send your Report of Medical History (mailed to you earlier with your Admissions Acceptance Packet) to: Mary S. Shook, ASU, Student Health Services, 614 Howard Street, PO Box 32070, Boone, NC 28608-2070. You may personally deliver your completed form to Mary S. Shook Student Health Services, 2nd floor – Miles Annas Student Services Building, between the hours of 8:00 – 11:30 a.m. and 1:00 – 4:00 p.m. when you come to Orientation.
