organ recipients can still die recipients have to take anti-rejection medication which is harmful to the body live donors also go through a lot of pain and if something should happen to them then they might be in trouble (say if they donated a kidney and something happened to their kidney that they have left) if live donor has unknown condition not known before transplant, the recipient gets the same disease and they would both need transplants depending on the condition  more
