What are my out-of-pocket costs with Advantra Freedom Medicare Private Fee-for-Service Plan?
• Your annual deductible is $100. This replaces the Medicare Part B deductible of $135. • You will have no additional out-of-pocket expenses for Medicare-covered services • You pay 20% coinsurance for plan approved services not covered by Medicare with an annual out-of-pocket maximum of $380 in addition to the $100 deductible. This benefit is the same as what you had under the PEBTF Major Medical.
