What are my responsibilities as a COBRA participant?
• Enroll within 60 days of your notification. • Remit your first premium payment within 45 days of your enrollment. • Pay your COBRA premium by the first of each month, even if you do not receive a bill. • Notify SHPS of divorce, legal separation, death of spouse or dependent, any disability or dependent child reaching age limit (varies by health plan).
