Literally the 4 D's = Dereliction of a Duty Directly causing Damages. (Dereliction meaning "deliberate or conscious neglect.") The 4 D's are used when a case is presented against a physician/practice for negligence/malpractice. The patient must provide proof against the physician for all 4 parts.
