What are the benefits of the multi disciplinary team for a service user?
Some answers here http://www.southwestcare.org/Medical_Ser… GP’s don’t normally form part of a multi-disciplinary team because of their workload. They do of course attend case conferences. Historically there has been tension between various care disciplines. A close knit team should in theory increase co-operation between all parties. Service users should have an individual care plan. The plan in a nursing home might be administered by the head nurse. In the community it would normally be drawn up by a social worker. The care plan sets out the needs of the service user, who and how those needs are to be met. Often a diary or communication book is used, so team members can stay informed of events and changes to the services users condition and needs..
