The following will be required for all nurse aide testing candidates: • An unexpired state or federal issued photo identification • A United States Social Security card that is not laminated; and • If the Social Security card states “Not Valid for employment without immigration and customs enforcement authorization” or contains a similar statement a final examination candidate shall present an Employment Authorization Document issued by the US Department of Homeland Security. • The name on the identification card and social security card must match.
