One of CAI’s ink tank batches that was mixing overnight generated a whole lot of vapor. The Chemical Safety Board (CSB) concluded that an employee must have forgotten to shut off a steam valve on said tank before he left for the day. Combined with CAI’s air ventilation system routinely being shut off overnight, the vapor filled the facility and was ignited, causing an explosion.
