If your address or the number of days per week you commute to work using qualifying transportation changes, you MUST complete a revised Public Transportation Subsidy Program Application form, have your supervisor approve/certify the form, and submit the completed application to your bureau or office coordinator for approval and processing by DOT. Use the regular form found at: http://www.nbc.gov/facilities/pdfs/PublicTransportationSubsidyProgramApplication.pdf and check the Revised Application box at the top of the application.
