WHAT ELEMENTS CAN THE PHARMACIST CHANGE ON A CII PRESCRIPTION?
– Per letter received from the DEA in October 2008, until the DEA issues additional rules on the subject, pharmacists are instructed to adhere to state regulations or policy regarding those changes that a pharmacist may make to a schedule II prescription after oral consultation with the prescriber. – Currently Wisconsin allows the pharmacist after consultation with the prescriber to add, modify or clarify the following per Phar 8.05 (7): – Strength – Dosage Form – Quantity – Date of issuance (Date written) – Directions for use – DEA Registration number of prescriber – Address of prescriber and patient – A pharmacist may not add, modify or clarify the following per Phar 8.05 (7): – Patient’s name – Product prescribed (except thru valid generic substitution) – Signature of prescriber – Wisconsin does not specify that a pharmacist can change the Dispense/Fill on Date written on a ladder prescription.
