What form is used by nominees, directors and staff to advise about changes to personal circumstances?
Under sections 141C and 141H of the Child Protection Act 1999, if there is a change in the personal, criminal or child protection history of a nominee, the nominee must immediately inform the Department of this change. Similarly, under sections 141D and 141I of the Act, if there is a change in the personal, criminal or child protection history of a director or staff member, the person must immediately disclose this to the nominee who in turn must notify the Department of this change. To advise the Department about changes in personal, criminal and child protection history, nominees, directors and staff of a licensed care service, are required to use the Licensing of Care Services (LCS) form 6, which is available on the Department’s website or from the relevant Regional Community Support Team.
