What forms are necessary when registering for service learning?
Two forms are needed when registering. The first form is the site placement sheet and the second is the materials release form. These forms may be located on this web page (click on downloadable forms) and are also available on page 16 and 17 of the service learning CD. Check the Webct. calendar. There will also be postings outside Zook 1. Instructors will also be notified of registration days and times.
