The Classroom Multimedia Support Services reservation (LS-1-1) and the UT Arlington “Removal of Equipment from University Campus” forms must be completed, signed and dated by the person responsible for the equipment. The reservation will be reviewed by the OIIR director or supervisor who will decide to approve or deny the request.
