What forms will be required from winning student contestants?

All student contestants under the age of 18 will be required to send in a form demonstrating that they had parental consent to participate in the contest. We will publish full instructions for claiming prizes and submitting the parental consent form before February 1, 2008. Winners may also be required to submit other paperwork as well.
