Failure to disclose all or part of your conviction(s) may be grounds for denial or disciplinary action as you falsified information required on your application for licensure or renewal of licensure. If you do not disclose an arrest resulting in charges being filed and/or resulting conviction, it is likely that your license will be denied or it may be issued subject to a period of probation (i.e. license restriction).
