When an outside party is requesting a certificate of insurance from Coastal Carolina University, please use the Certificate Request Form. Forward all completed certificate requests to hmassey@coastal.edu or fax to the Office of Risk Management(843-349-2384) for processing. Please allow 3 to 5 days for processing.
