Students must complete the entire Health History Form including Part III Physical Exam section. Students in these programs will not be allowed to participate in their clinical or fieldwork practice unless a preadmission physical examination is on file in the Health Office. There are also additional requirements for these students: • Tuberculin Skin Test – Two doses of PPD Mantoux testing given at least one week apart is REQUIRED for Nursing, PTA and Dental Hygiene. Only one PPD test is REQUIRED for Early Childhood. This is not a vaccine but a test to determine previous exposure to tuberculosis. • Proof of measles, mumps, and rubella immunity as required for all college students. Note: Students in Nursing, Occupational Therapy Assistant, PTA and Dental Hygiene born before January 1, 1957 must show proof of immunity to rubella only. • Students will also need to call the Health Center and make an appointment with a nurse in order to complete a health clearance for clinical or fieldwork. ...  more
