What if I miss the September 16, 2009 deadline or am on vacation August 24 – September 16?
Because the Questionnaire is available from any computer with an internet connection, if you do not complete the Health Risk Questionnaire by September 16, 2009 Midnight Central Time, you will pay higher contributions for Motorola Health Plans (Health Investment Plan, Health Advantage Plan, and Health Plan Plus) coverage in 2010. If you did not submit the HRQ before the September 16 deadline, there will be a second HRQ period from September 23 until midnight Central Time January 18, 2010. If the HRQ is completed during this time, you will be eligible for discounted monthly contribution rates starting with your first February 2010 paycheck deduction. You will miss the discounted rate for one month only (January).
