As the settlement class consists of former and present owners, checks were issued to class members under the owner accounts paid by ConocoPhillips through time. For this reason, checks may have been issued under the former names, address, and/or tax identification numbers of class members. If you are able to cash your settlement check, you may not need to do anything. If you are unable to cash your check, then you will need to have your check reissued. Due to tax reporting requirements, the Settlement Administrator cannot reissue a check automatically if there is a request for a change in the name, address and/or tax identification number of the payee. Class members who want their settlement check reissued to reflect a change in their own name, address or tax identification number should submit a Check Reissue Request Form to the Settlement Administrator.
