What is a Personnel Activity Report?
This form is required by the University for academic and staff employees who are paid from a federal source regardless of the percent of time. The form is certified quarterly and fulfills effort reporting requirements specified in OMB Circular A-21. This form provides after-the-fact certification of salaries and wages charged to federal funds. Salary charges not certified through this form are subject to disallowance. The form may be completed by the employee or responsible official (supervisor) who will certify the effort on the various funding sources. The supervisor is a person who has comprehensive and first-hand knowledge of where the employee spent his/her time. For academic employees, the responsible official must be a person with an academic title code.
