A Pre-Existing Condition Limitation is defined as: 1. The existence of symptoms which would cause an ordinarily prudent person to seek diagnosis, care or treatment within the 12 months immediately prior to the Insured’s Effective Date under the policy. 2. Any condition which originates, is diagnosed, treated or recommended for treatment within the 12 months immediately prior to the Insured’s Effective Date under the policy. Reinstatement application must be made in person at the Student Insurance office. If you are not able to come to the Student Insurance office to make application, please contact us by e-mail at insure@uiuc.edu and arrangements will be made.

(1) Medical advice, Consultation, care or treatment was recommended by, or received from, a Doctor within 12-months immediately prior to the Effective Date of coverage for a Covered Person; or (2) Symptoms existed within 12-months immediately prior to the Effective Date of coverage for a Covered Person that would cause a reasonable person to seek Consultation, care, or treatment from a Doctor. Consultation means evaluation, diagnosis, or medical advice given without the necessity of a personal examination or visit.
