What is a USPS Form 1583 and why do I need to fill out at least one?
We are registered with the U.S. Postal Service as a Commercial Mail Receiving Agency (CMRA), and our customers are required to complete a USPS Form 1583 to authorize us to receive mail on their behalf. This same form is used by the USPS when you open a P.O. Box. You will need to send to Earth Class Mail at least one completed and notarized 1583 form for each Earth Class Mail address in your account. An additional 1583 form is needed for each recipient at an Earth Class Mail address who is not an employee (for business accounts only), a spouse, or a minor son or daughter living at the same residential address. Likewise, an additional 1583 form is needed for each business name that will receive mail at an Earth Class Mail address. (Customers outside the United States who are using our Portland or Beaverton addresses do not need to have their forms notarized.).
