What is Healthydirections and how does disease management benefit me?
Healthydirections® is a disease management program the FSBP offers to members and covered dependents with asthma, chronic obstructive pulmonary disease (COPD), coronary artery disease (CAD), diabetes, or heart failure (HF). The program may help you or your covered dependent improve in health at no additional cost to you. The Plan identifies candidates using their medical and prescription drug claims information. The Program will contact the candidates and offer them the opportunity to participate. Participation is strictly voluntary.
