Simulation is part of the radiation therapy planning phase that takes place in a separate appointment before any radiation treatment begins. The patient is asked to lie very still on a table for up to an hour, and may need to wear a specially fitted mask, frame or board to immobilize the part of the body that is being mapped for treatment. While the patient lies still, a radiation therapist (a specially trained technologist) under the supervision of the radiation oncologist uses a special x-ray machine to pinpoint the treatment area, sometimes called the treatment portal or field. This is the exact place in the body where the radiation will be aimed. Ink lines are usually drawn on the skin to mark the area to be treated. Tattoos (tiny permanent skin markings the size of freckles) may also be placed at the time of simulation.

The term simulation is used in different ways by different people. As used here, simulation is defined as the process of creating a model (i.e., an abstract representation or facsimile) of an existing or proposed system (e.g., a project, a business, a mine, a watershed, a forest, the organs in your body) in order to identify and understand those factors which control the system and/or to predict (forecast) the future behavior of the system. Almost any system which can be quantitatively described using equations and/or rules can be simulated. The underlying purpose of simulation is to shed light on the underlying mechanisms that control the behavior of a system. The design is modeled, often starting with CAD data, within a simulation product suite like Femap on a PC. The simulation model is then virtually tested, using an FEA solver like NX Nastran, to verify that the design will function as expected under defined operating conditions. Simulations, or planning sessions, are procedures that a radiation oncologist and patient schedule prior to beginning a course of radiation treatments. At this session, members of the radiation oncology team make devices which allow the patient to be treated in precisely the same position for each treatment. Images are made and measurements are taken which permit the calculation and delivery of precise, accurate, individualized treatments. Discrete Event Simulation A simulation is a computer model that mimics the operation of a real or proposed system, such as the day-to-day operation of a bank, or the running of an assembly line in a factory, or the staff assignment of a hospital or call center. The model is time based, and takes into account all the resources and constraints involved, and the way these things interact with each other as time passes. Simulation also builds in the randomness you would see in real life. For example it doesn't always take exactly 5 minutes for a customer to be served and a customer doesn't always arrive every 15 minutes. This means that the model really can match reality - so something you try in the model will behave the same way as it would in real life.

Computer simulation is the discipline of designing a model of an actual or theoretical physical system, executing the model on a digital computer, and analyzing the execution output. Simulation embodies the principle of ``learning by doing'' --- to learn about the system we must first build a model of some sort and then operate the model. The use of simulation is an activity that is as natural as a child who role plays. Children understand the world around them by simulating (with toys and figurines) most of their interactions with other people, animals and objects. As adults, we lose some of this childlike behavior but recapture it later on through computer simulation. To understand reality and all of its complexity, we must build artificial objects and dynamically act out roles with them. Computer simulation is the electronic equivalent of this type of role playing and it serves to drive synthetic environments and virtual worlds.
