All photographers who are certified either by Wilmer, Wisconsin, Duke or DARC reading center(s) for both OCT and FA on any macular edema study (regardless of the cause), will be grand fathered based on their current certification. However, sites must provide us with the documentation pertaining to certification. Please download this form and fill it to send along with documents for certification. Please submit it to cert@read2.net or fax it to 410-502-8958 ATT: READ2 CERTIFICATION.
