What is the difference between a medical record and a Personal Health Record?
A Personal Health Record includes details about a patient’s health history, and current health situation, with information about treatments and medicines, vaccinations and other relevant facts. Over the years a patient’s Personal Health Record likely will be contained in miscellaneous manila folders with the doctors he/she has seen over the years or hospitals and other health care facilities that he/she has visited. Only by preparing his/her own Personal Health Record and checking all facts can a patient be assured that his/her personal health record is accurate.
