Based on society’s reaction i guess, and perhaps the outlook on life of those making the distinction If i was working in a hospital and i reported them for neglect of patients, i doubt anyone would call me a grass. If i was in prison, and told a guard that another prisoner was dealing drugs (particularly if that disclosure would gain me brownie points) i would likely be called a grass by those who have an affiliation/sympathy/tolerance for drug use, but others may well regard it as whistle-blowing. If i called the police on my sister with multiple-sclerosis who self-medicated with cannabis, i doubt that there would be much sympathy from anyone, and most would condemn me as a grass, amongst other names.
