If your plan has two levels of coverage for In-Network(INN) or Out-of-Network(OON), there can be large difference in your cost. Example for XYZ Insurance: INN specialist visit is covered with a $25 copayment. OON specialist is covered at 80% coinsurance. If you see an INN specialist, you will have to pay a $25.00 copayment to the specialist at your visit. There is no additional charge to you. If you see an OON specialist, you will have to pay 20% of the Usual and Customary rate(U&C), plus the difference between the billed charge and the U&C insurance payment.
