If you have tickets for more than one mode of transport, for example, a BlueTravel Pass and a FerryTen, you will need to complete 2 forms – one for the BlueTravelPass (bus) and one for the FerryTen ticket. When you have completed and signed your form(s), enclose your original ticket(s) and place in an envelope(s) addressed to: State Transit Authority Ticket Distribution Reply Paid 2132 STRAWBERRY HILLS NSW 2012 No postage stamp is required if you address the envelope this way. You may wish to make a photocopy of your form and your original ticket(s) before posting.
