What is the penalty for using a non-preferred provider (outside the Municipality of Anchorage)?
The Plan uses Beech Street providers nationwide. If you use a facility that is not Beech Street and it is within 25 miles of a Beech Street provider, the Plan will reduce your reimbursement rate by 20%.
