• a summary of dates of contact with the injured employee and with other pertinent parties; • a discussion of problems or issues affecting the employment process and corrective actions taken; • the continuing likelihood of success of the rehabilitation plan; • the vocational activities planned for the next month; and, • justification for any change in the plan.
